Update Chapter Contact Info

Please use the webform below to update your club's contact information.

If your club has an updated membership roster to submit, click HERE to e-mail it to us. Please attach your updated membership roster to the e-mail. We need a new roster once a year, or whenever membership changes. This helps us determine how many patches to send when a donation is made and it helps us to be able to identify Good Sam members when individual donations are sent in for year-end award credit.

Note: The chapter contact is the person who will receive our print version of the Canine Listener magazine, credit notifications, and the person we will call if we have any questions. Donation acknowledgements will be sent to the officer who sends in the donation.
